* First-time patients must bring two forms of ID: NHI card + photo ID (driver's license, national ID, or passport). Minors can use student ID or household registration instead.
* Major Illness Cards and Disability Manuals must be for psychiatric diagnosis and within valid dates to qualify for discounts.
* Major Illness Card, Disability Manual, and Veterans must present the physical paper document at registration and sign the discount form. Without it, you'll be charged as a general patient.
* Medical certificates require discussion with your doctor. Cost: NT$200 per certificate. Medical record copies: NT$5 per page.
Getting Care: What You Need to Know
1. Registration: The Basics
- What to bring: Your NHI card and a photo ID (like your driver's license or national ID). If you're under 18, bring your household registration instead.
- How to register: Walk-in only - no advance booking required.
- When to check in:
- First-time patients: Check in at least 30 minutes before we close.
- Returning patients: Check in at least 15 minutes before we close.
- ❗️ Early cutoff notice: To keep care quality high, we may stop taking new patients early if there are more than 12 people waiting (or more than 5 new patients) within the last hour before closing.
2. For Patients Under 18
- First visits: If your parents can't come with you, you'll need to fill out a medical authorization form and parental consent form beforehand.
Download the forms here - ADHD treatment: If you've seen a psychiatrist elsewhere between ages 6-18, please bring those records. After evaluation and additional assessments (like CPT, ASRS), if long-acting medication is needed, we'll submit a pre-approval application to the health bureau per the August 2022 regulations.
3. Medications & Prescriptions
- In-person visits required: By law, doctors must see you in person before prescribing. Exception: If you have mobility issues, cognitive impairments, or dementia, someone else can explain your condition and pick up medications.
- No long-term prescriptions: We don't provide chronic disease continuous prescription forms.
- Duplicate prescriptions: The health bureau checks for duplicate prescriptions across hospitals. If our system shows you still have medications left, we can't prescribe duplicates. Official notice | 2018 guidelines
- Sleeping pill regulations (Zolpidem / Zopiclone):
- Haven't visited in 6 months: You'll be treated as a new patient and can only get a 7-day supply.
- If you've sleepwalked before: Doctors cannot prescribe these medications to anyone who's had complex sleep behaviors (like sleepwalking or sleep-driving).
4. Other Things to Know
- If you need more time for in-depth support, ask your doctor about counseling or psychotherapy sessions.
